How do you plan to pay for your evaluation?

Please note: We are currently in-network with Aetna, Blue Cross/Blue Shield LA, Cigna, Optum, Oscar, Oxford commercial plans only (No Medicaid, Medicare, or Tricare accepted at this time). If your insurance is not listed, consider signing up for our reduced cost monthly subscription plan.

I understand that out-of-pocket payments (Co-pays or Subscription plans) are due at the time of my appointment.
